About the procedure
When a standard extraction isn't enough.
Some teeth cannot be removed using standard forceps. Impacted wisdom teeth, teeth with fractured or curved roots, root tips, and teeth that are associated with cysts or other pathology all require a surgical approach - carried out by an experienced specialist.
Ms. Soudeh Chegini performs complex extractions with a level of care and precision that minimises trauma, reduces recovery time, and achieves the best possible outcome. Detailed pre-operative planning using digital X-rays and CBCT imaging means every procedure is prepared for thoroughly.
Intravenous sedation is available for patients who are anxious or for whom the procedure is particularly extensive, making even the most complex extractions as comfortable as possible.

We review your medical history, examine the tooth, and take any imaging needed (OPG or CBCT) to plan the procedure thoroughly.

Ms. Chegini explains the procedure in full, including the technique, risks, and aftercare, and obtains your informed consent.

Anaesthetic and sedation
Local anaesthetic is administered. Intravenous sedation is available for anxious patients or particularly complex procedures.

Surgical extraction
The tooth is removed using appropriate surgical technique - which may involve bone removal, sectioning the tooth, or suturing the socket.

Post-operative care
We apply haemostatic material and sutures where needed, and ensure bleeding is controlled before you leave the practice.

A follow-up appointment is arranged to check healing, remove sutures if placed, and address any concerns.

What makes an extraction 'complex'?
An extraction becomes complex when the tooth cannot be removed using standard forceps alone. This may be because it is impacted, has unusual root anatomy, is fractured, or is closely related to important anatomical structures such as the inferior alveolar nerve or maxillary sinus.
Is IV sedation available?
Yes. Intravenous sedation is available for patients who are anxious about the procedure or for cases where the complexity warrants a more controlled environment. You will need someone to accompany you and drive you home.
How long is the recovery?
Recovery depends on the complexity of the procedure. Simple surgical extractions typically settle within three to five days. Impacted wisdom teeth may take five to seven days for the acute phase to pass, with complete healing over several weeks.
Will I feel anything during the extraction?
No. Local anaesthetic ensures the procedure is pain-free. You may feel pressure and movement, but not pain. If at any point you feel discomfort, please let Ms. Chegini know immediately.
What aftercare will I need?
You'll receive written aftercare instructions covering diet, oral hygiene, pain management, and what to do if complications arise. Antibiotics and appropriate pain relief are prescribed when needed.
